ISAAC Phase Three Centre Report

The ISAAC Centre Report has recently been revised to make this document easier to complete, especially for those centres that undertook Phase One. We have included a copy of the revised Centre Report on the website, to enable investigators to become familiar with the format of the report, while awaiting a copy to be sent from the ISAAC International Data Centre (IIDC).

The Phase Three Report is either emailed or posted to individual centres from the IIDC when the Registration process has been completed - that is following approval from the appropriate National and Regional Coordinators and on receipt of a completed Registration Document by the IIDC.

The Phase Three Centre Report includes detailed questions concerning the methods you used during your Phase Three study. If the completed centre report can be submitted to Philippa Ellwood (p.ellwood@auckland.ac.nz) at the same time the data is submitted to the IIDC, this will enable the checking process to be undertaken without delay. If centres prefer to post this to the IIDC, the postal address is:

You may encounter a problem when downloading the Microsoft Word® versions of the Centre Reports. If your web browser does not offer you the opportunity to save the files or view them in Microsoft Word®, but instead displays text and/or meaningless symbols, you can manually download the file by following these instructions:

  1. Use your right mouse button to click while the mouse is postioned over the link (e.g. "Centre Report, 6-7yr (Word)").

  2. Select the "Save Link As..." option from the pop up menu that has appeared.

  3. Select an appropriate location and click on the "Save" button to save the file in the normal manner.
